Star of the Sea Community Care offer Support at Home, Veterans Home Care and Private Packages for older people who wish to stay in their own home longer.

Star of the Sea Community Care is a holistic, vibrant aged care service based in Wallaroo, providing Support at Home, Veterans Home Care and Private Services by Aged Care Operations for the Catholic Diocese of Port Pirie with the purpose of improving the lives and wellbeing of older people in our community.

Our registered aged care service provides support and care designed by you, and delivered by a professional team of highly-trained care workers in the spirit of true friendship.

Community Care offers a selection of services assigned through Support at Home, which enables older people to choose the types of care and services that best suit their particular assessed needs.

Community Care is inclusive of veterans’ home care services approved by DVA, and exclusive, private services at a negotiated fee.

Your home. Our care.

Support at Home is tailored, co-ordinated services that meet your individual needs and goals based on an assessed Support Plan created by the Department of Health, Disabilities and Ageing.

Eligibility for a Government funded package is assessed by an Aged Care Needs Assessor and Star of the Sea Community Care is able to assist with applications for this initial requirement.

Our Community Care workers will meet with you to discuss the types of care, and services and equipment which can be provided to enhance independence at home.

Each Support at Home participant receives set funding to meet their assessed needs. You can use this, alongside other funding sources, for Star of the Sea Community Care to provide tailored services.

Classification
You will receive one of 8 funding classifications for ongoing services. Your classification is determined in your aged care assessment. 
Quarterly Budget
Budgets are allocated quarterly. You can carry over unspent funds up to $1,000 or 10% (whichever is greater) to the next quarter.
Reassessment
If your existing budget cannot meet your needs, you may need to be reassessed to receive a higher classification and budget.
Private Services
You can enter into a private agreement with us for additional services, outside your budget, however you will pay for the total cost of these extra services.
My Aged Care Assessment and Support Plan
Following approval for Support at Home you will receive a Notice of Decision and your support plan to share with your provider. This will include your classification and your budget.
Short Term Services
Different funding amounts and classifications apply for participants accessing short term pathways:

– Assistive Technology and Home Modifications (AT-HM) scheme has separate needs-based funding for 12 months (in most instances).
– Restorative Care Pathway funding for up to 16 weeks.
– End-of-Life Pathway funding.

Care Management Funding
For ongoing services, 10% is deducted from your quarterly budget to fund care management activities. The care management supplement funds additional support for specific participant groups.
Transitioned Home Care Package's
If you were transitioned from the Home Care Packages (HCP) Program you will receive a funding level for ongoing services that’s equivalent to your previous funding.
Older people assessed as eligible for a HCP as of 31 October 2025 were transitioned to the Support at Home Priority System.

Older People are able to retain independence in their family home and the community, yet still have the best caring support to suit their needs with choice and flexibility.

Support at Home Services:

Clinical supports

Specialised services to maintain or regain functional and/or cognitive capabilities. Services must be delivered directly, or be supervised, by university qualified or accredited health professionals trained in the use of evidence-based prevention, diagnosis, treatment and management practices to deliver safe and quality care to older people. Includes Nursing Care, Allied health and other therapeutic services, Nutrition, Care management and Restorative care management

Independence

Support delivered to older people to help them manage activities of daily living and the loss of skills required to live independently. Includes Personal Care, Social support and community engagement, Therapeutic services for independent living, Respite, Transport, Assistive technology and home modifications.

Everyday living

Support to assist older people to keep their home in a liveable state in order to enable them to stay independent in their homes. Including Domestic assistance, Home maintenance and repairs and Meals.
